MODULE 1.

THE MOST WIDESPREAD SOMATIC DISEASES IN CHILDREN.


AIMS OF THE MODULE.


To be able:


  1. To determine the etiologic and pathogenetic factors of the most widespread somatic diseases of children.

  2. To classify and analyse the typical clinical picture of the most widespread somatic diseases of children.

  3. To make the plan of observation and analyse data of laboratory and instrumental investigation at typical course of the most widespread somatic diseases of children.

  4. To demonstrate mastering the principles of treatment, rehabilitation and prophylaxis of the most widespread somatic diseases of children.

  5. To diagnose and to manage emergency care for children with the most widespread somatic diseases.

  6. To carry out the estimation of prognosis at the most widespread somatic diseases of children.

  7. To demonstrate mastering the principles of medical ethics and the system of professional seniority in paediatrics.

PRACTICE 1

Subject: Diathesis. Allergic rhinitis. Atopic dermatitis. Urticaria.


Importance of the Subject: Сonstitution's anomalies (diathesis) are the states when organism's functions are in a state of unsteady equilibrium; when an organism has some individual congenital, hereditary and sometimes acquired characteristics. They predispose an organism to pathologic reactions at external harmfulness, and make it predisposed to particular diseases and to their severe course. Getting each of the constitution's anomalies is reasonable. These are concrete conditions in infants, which demand special approach to organization of diet, regimen, vaccinations, treatment of diseases appeared on the ground of diathesis.

Rhinitis describes diseases that involve inflammation of the nasal epithelium and is characterized by sneezing, itching, rhinorrhea, and congestion. There are many different causes of rhinitis in children, but approximately half of all cases of rhinitis are caused by allergies.

The precise mechanism producing atopic dermatitis, a form of eczema, is unclear; genetics, environmental factors, and cutaneous immunologic factors play a role. Children with primary T cell immunodeficiency disorders, such as Wiskott-Aldrich and Omenn syndromes, frequently have atopic dermatitis. When these children undergo stem cell transplantation, the procedure corrects the immunologic defect and the dermatitis.

Urticaria and angioedema are caused by an allergic, IgE-mediated reaction to an allergen that activates mast cells in the skin.


Aims of the Practice:

Goal to gain knowledge and develop skills in the following: diagnosing diathesis, allergic rhinitis, atopic dermatitis, and urticaria, developing a treatment plan, and deciding on appropriate preventive measures.


Skills To Be Mastered

Prerequisite Knowledge and Skills

1. Find among symptoms and patient case history indications of diathesis, allergic rhinitis, atopic dermatitis, and urticaria.

1. Interview the patient on his complaints and history of the disease (as explained in the Propaedeutic Pediatrics Course).

2. Develop a target plan to diagnose diseases under study.

2. Decide on the scope of patient examination, including the particular techniques to be used and their sequence, and (as explained in the Propaedeutic Pediatrics Course).




3. Identify indications of diathesis, allergic rhinitis, atopic dermatitis, and urticaria during objective examination of a patient and during laboratory tests.

2. Examine the physical condition of a patient and identify normal features and abnormal changes. Explain the outcomes of the examination tests as indicated: laboratory tests and instrument-based examinations (as explained in the Propaedeutic Pediatrics Course).

4. Decide on a treatment plan for patients with diathesis, allergic rhinitis, atopic dermatitis, and urticaria.

3. Provide reasons for the categories and types of medications prescribed, and explain individual and public preventive measures. (as explained in the Medical Drugs Course).

5. To demonstrate mastering the principles of medical ethics of socializing with a child and his parents.
